CARE PATH - THE 12 STEPS

A care path for a complex pathology is a sophisticated process which can last from a few weeks to several years. 

It begins the moment the patient, the governmental body or his insurance company make contact with C3Medical and continues until the return home, where the follow-up process then takes over.

For each of the 12 key steps, C3Medical will be present and will provide active support throughout.
 

STEP 1 - INITIAL CONTACT AND UPLOAD OF MEDICAL FILES

Here begins the preparation of the decision making process where the medical specialists will decide whether or not to welcome the patient in France.

Soon after the initial contact and the signing of a letter authorizing the treatment of personal, medical and ID data, the C3Medical Medical Patient team will upload all the necessary medical documents (eg. records, DICOM files, etc.) into C3Medical MGPS BestCare digital platform. 

All the documents will be instantly transferred to the relevant specialist(s) or hospital(s) for a Second Opinion, or an RCP in case of a cancer. 
 

STEP 2 - SECOND OPINION AND RCP

If no additional elements are requested by the French doctors (eg. MRI, scans, block of tumor shipment, biopsy to be completed), the decision based on the diagnostic will be known within a few hours (vital emergency) to a week / 10 days - in case of a RCP (“Multidisciplinary Consultation Meeting” in English).

Made mandatory by French law when dealing with cancer cases, these weekly RCP (MCM) bring together health professionals duly selected by law for their skills and level of experience with cancer cases.  During these weekly MCM a patient's situation is discussed, the possible treatments according to the latest scientific studies, analysis of the benefits and the risks incurred, as well as the evaluation of the consequences to the quality of life which will result. 

MCM bring together at least 3 different specialists. More often: medical oncologist, radiation oncologist and surgeon. The lead doctor then informs his patient and explains to him and his relatives, in detail, his personalized care plan in order he/they can play an active part in the healing process.

Fyi., the MCM - a  “simple” change in the daily medical organization - has helped to boost the efficiency rate in cancer treatments in France over the last two decades.
 

STEP 3 - CARE PATH PROPOSAL

Following the 2nd Opinion, if a positive go-ahead is given by the doctors, a care path proposal is presented in less than 24 hours by the C3Medical Patient Assistant. 

The proposal includes all the relevant details the patient needs to know, including all the necessary assistance, logistics, the housing, etc. (see “Services” section) for the duration of the care path, along with a precise and detailed budget plan.

The C3Medical Patient Assistant will present the proposal and answer any queries either through a meeting at the office or through a conference call if you, the patient, are not close by. 

At this time, the patient – or his insurance – can also ask for specific or additional services such as a VIP treatment for you and your family during the stay (eg. transfer via private jet, VIP housing, additional services, personal assistant or protection), etc. 

Supported by the C3Medical Conciergerie Service, the Patient Assistant will be glad to provide you with an additional proposal within few hours.        
 

STEP 4 - CONTRACT SIGNATURE

After explaining and presenting the care path, the terms & conditions and some additional information regarding the hospitals and doctors, the signing of the contract officially triggers the beginning of the care path process. 

STEP 5 - PAYMENT

As soon as the contract is signed, you proceed with the full upfront payment: 100% payment of the care path costs (care + accommodation) is mandatory as requested by the French authorities to establish your medical visa.

The payment is either made to the C3Medical bank account or directly to the hospital(s) or the doctor(s) if the activity is directly managed by C3Medical (e.g. Institut Curie or IMM / Institut Mutualiste Montsouris).

C3Medical cannot accept partial or differed payment.
 

STEP 6 - VISA AND TRAVEL PREPARATION

Whatever your case, its level of emergency and your nationality, the different formalities will be explained by your Patient Assistant, with support from C3Medical if needed.

In some cases, if you are a citizen from a country which has set bilateral agreements with France / EU, a visa may not be necessary.

If a visa is required, and once payment has been made, C3Medical will issue an Invitation Letter. The visa application process can then begin at your local French Embassy or Consulate. 

Thanks to the support of the doctor at the French Consulate, our teams will speed-up the usual lengthy and complicated process to obtain the required authorization + legal documents for your long stay in France. 

Both are issued by the local French Consulate or Embassy and the Immigration Authorities in Paris. In most cases, this process takes from 2 days up to a week. 

In case of a vital emergency, with the support of the doctor at the French Consulate or Embassy, the delay can be reduced to less than 24 hours.

In the meantime, our local C3Medical team will begin organizing your stay in France, and do all that is necessary for you and your family to be comfortable.

Note:  We do not book your flight tickets: unless you request VIP service (see "Services").  Booking your flights will be in your hands as you may have specific demands or airline company preferences.
 

STEP 7 - YOUR ARRIVAL IN FRANCE

Upon your arrival at Paris/Charles de Gaulle Airport - or at the closest regional airport -, a C3Medical Care Manager will be there to meet you at the luggage delivery lounge exit. 

The C3Medical Care Manager will greet and take care of you, driving you to your hotel or your appartment, providing you with useful local information leaflets, even accompany you to the closest store and where you can find some food or buy some essentials.

From this day on, up to the date of your flight back to your country, your Care Manager will be your main contact, in addition to 24/7 assistance from our teams, to make your stay in France as pleasant as possible and – if necessary - to assist you in any last minute requests. 

Throughout your stay, you will be treated with care and consideration by the hospitals as well as the C3Medical teams.

You and your relatives will be constantly treated as human beings, as individuals to be respected for their own rights, just as if you were French citizens.
 

STEP 8 - REGISTRATION AT THE HOSPITAL

Shortly after – or the same day if possible, or in case of emergency -, you will complete the registration process at the hospital with the help of your Care Manager and - if needed – an interpreter.

STEP 9 - FIRST MEDICAL APPOINTMENT WITH THE LEAD DOCTOR

In general, the day after your arrival, you’ll have the first medical appointment with your lead doctor, who will be your referent doctor throughout the care path in France.

Your C3Medical Care manager will be with when you meet your lead doctor and will also introduce you to the rest of the medical team - eg. other doctors, nurses, paramedics - who will take care of you during your care path. 

The lead doctor will then perform a close examination to readjust treatment, if necessary, and eventually ask for some additional tests to determine the illness progression and begin the care process without delay.
 

STEP 10 - CARE PATH ROLL-OUT AND C3MEDICAL SUPPORT

After the first medical consultation, you’ll have a clearer picture of your treatment timetable: precise duration, planned agenda etc. 

To help, you will have access 24/7 to all your relevant information through your cell phone, thanks to MGPS BestCare information and communication digital platform. 

Your doctor, your hospital, your family or any contact you want to be informed about your situation, etc., will benefit from a personal access to get the information they need to know.  

Through each major step of your treatment, the Care Manager will be by your side. You will regularly receive phone calls, messages or visits from different C3Medical team members, to get your feedback, and to make sure you are comfortable.

You’ll never be left alone, even if you don't ask us to be there with you. 

In case of emergency – medical or whatsoever -, you or your relatives can access C3Medical 24/7 Assistance.  
 

STEP 11 - END OF YOUR CARE PATH IN FRANCE

At the end of your treatment you will  have a debriefing meeting with your C3Medical Care Manager and some of the active team members and partners from your stay. They will welcome any comments and suggestions you may have for improvement. 

In addition, the C3Medical Financial and Controlling Department will issue the final financial balance – difference between the payments and the costs. 

In case of leftover budget, C3Medical will reimburse this to you 100% in a matter of days, pending the reimbursement to C3Medical of the excess payments by the hospital(s) or doctor(s) -.  

In addition, you or your insurance company will receive a copy of all  information documents related to your medical treatment in France. 

You will also have on-line access, 24/7 to your personal information, stored on C3Medical MGPS BestCare digital platform, even years later.
 

STEP 12 - POST CARE FOLLOW UP

The C3Medical team in charge of you will also keep in touch, thanks to regular reminders triggered by our information system: just to know how you are, in case you need further assistance.

 In the same light; to keep you updated - if you agree -, you will receive regular updates and news from C3Medical and French healthcare. 

In the event you will need regular long-term treatment, we will provide you with a plan of the next steps and appointments, and remind you in advance, with emails and sms, not to miss them – through C3Medical MGPS BestCare digital platform.

To make your life easier, we will encourage these treatments or checks to be performed in your own country, in selected highly qualified hospitals or with C3Medical referents, to avoid the flight time, the costs and the (possible) formalities associated to a new trip to France.

If such an option is not possible because of the lack of local facilities or expertise, or if you prefer to have care in France, C3Medical teams will organize your next medical trip to France, on the same basis as has already been described.
